The South West Integrated Personalised Care (SWIPC) Awards 2024, held at The Bishop's Palace in Wells on February 27th, brought together a multitude of distinguished guests to celebrate excellence in integrated and personalized care. Among the honorees was Meaningful Measures Ltd., who, in partnership with NHS Cornwall and Isles of Scilly community health and wellbeing teams, received the prestigious health equity award.

The award recognized the impactful work of the "Community Health and Wellbeing Workers" (CHWW) program, launched by Cornwall and Isles of Scilly Integrated Care System in February 2023. This initiative was designed to address health inequities by embedding teams of CHWWs within underserved communities to build relationships, identify needs, and provide tailored support.

The CHWWs' approach centers on empowering individuals to identify their primary concern and then collaborating with them to address and alleviate that concern. By utilizing the Measure Yourself Concerns and Wellbeing® (MYCaW) tool, the program effectively captures outcomes related to social determinants of health and overall well-being, thereby shedding light on the pervasive inequities experienced by these communities.

The initial success of the pilot program led to its expansion into Penwith, Bodmin, and Liskeard, demonstrating its efficacy in fostering equitable health outcomes. The recognition bestowed upon the CHWW program at the SWIPC Awards 2024 underscores its significant contribution to advancing health equity within the region.
