Application form for Licence to use Measure Yourself Medical Outcomes Profile (MYMOP®).

How to get your MYMOP® licence:

  • Review our license policy.
  • Complete and submit the application form below and we'll review your request within 3 business days.
  • You will either be offered a 12-month license or asked for further information.
  • You'll receive an invoice for payment.
  • Upon payment, we'll provide the MYMOP® licence, tool, guidance, and supporting documents. Please note, licence payments are non-refundable.

Please note we offer special deals to large NHS bodies (Integrative Care System level), and also Complementary and Integrative Health Care Associations click for more info.
